Established in 2024 to provide patient focused, goal-oriented Case Managers and Case Management services across Wales.
Our team is based in Llantrisant and offers introduction to experienced Case Managers with clinical backgrounds who will support your clients with managing the journey towards their goals. We offer Immediate Needs Assessments prior to any placement to provide a detailed overview of your clients’ current situation with a view to helping individuals to set measurable and achievable goals for their future.
We liaise with key stakeholders to provide regular updates on any ongoing cases. We are Care Inspectorate Wales registered and operate within safer recruitment guidelines to ensure that you can always feel confident and assured that the service you receive is second to none.
Our Case Managers have vast experience working as part of a Multi-Disciplinary Teams, their patient focused planning and support ensures that you are always able to take steps forward towards a future that reflects your aspirations. As well as offering supportive, caring and experienced Case Management staff, we have a wider network of support functions including our staffing partner, MPS Complex Care.
